Using 3M PPRs & PPCs to Measure the Triple Aim

3M quality measures are used by hospital associations, Hospital Engagement Networks (HENs) and state Medicaid agencies for public reporting of readmissions and complications. Why? Because they accurately measure rates of potentially preventable readmissions (PPRs) and potentially preventable complications (PPCs) based on risk-adjusted severity for a broad array of diagnosis categories, including conditions specific to mothers and children.

Your hospital may already be receiving reports from your hospital association or state Medicaid agency to show its performance against benchmarks. But the aggregated report data is only the “tip of the iceberg.” 3M PPRs and PPCs can also be used to look closely by case, service line or provider as an integral part of a patient safety program to:

  • Gauge trends against program goals quarterly or monthly
  • Identify areas of “impactibility” by diagnosis and severity level
  • Communicate risk of readmission and complication in clinically relevant terms
  • Estimate costs and reimbursements in line with payer reimbursement schemes
